Cal Arundel Family Medicine Policies

Insurance

We participate in most insurance plans, including Medicare. If you are not insured with a plan we participate in, payment in full is expected at each visit. If you are insured by a plan we participate in but do not have an up-to-date insurance card, payment in full for each visit is required until we can verify your coverage. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding your coverage. If your plan requires you to select a PCP (Primary Care Provider), please make sure that Dr. Wisniewski's name appears on your card. If Dr. Wisniewski has not been chosen by you as your PCP, payment for your visit will be expected at the time of service.

Workman's Compensation

Please bring your claim number and Workman's Compensation carrier information with you for your visit. If you do not have this information, your appointment will have to be rescheduled.

Motor Vehicle Accident Claims

As above, please bring your claim number and Motor Vehicle Insurance carrier information with you to your visit.

Co-Payments and Deductibles

All co-payments and deductibles must be paid at the time of service. This arrangement is part of your contract with your insurance company. Failure on our part to collect co-payments and deductibles from patients can be considered fraud. Please help us in upholding the law by paying your co-payments at each visit.

Non-Covered Services

Please be aware that some and perhaps all, of the services you receive may not be covered or considered reasonable or necessary by Medicare or other insurers. You must pay for these services in full at the time of your visit.

Proof of Insurance

All patients must complete our patient information form before seeing the doctor. We must obtain a copy of your current valid insurance card. If you fail to provide us with proof of insurance in a timely manner, you may be responsible for the balance of the claim.

Claim Submission

We will submit your insurance claims for you and assist you in any way we reasonably can to help get valid claims paid. Your insurance company may need you to supply certain insurance information directly. It is your responsibility to comply with their request. Please be aware that the balance of your claim is your responsibility whether or not your insurance company pays your claim, except in the instance where we are an in-network, participating provider. Your insurance benefit is a contract between you and your insurance carrier, and we are not a party to that contract.

Coverage and Demographic Changes

If your insurance changes, please notify us before your next visit, so that we can make the appropriate changes to help you receive your maximum benefits. If your insurance company does not pay your claim in 45 days, the balance will be automatically billed to you. If your address or telephone number changes, please also add Cal Arundel Family Medicine to the list of address change notifications.

Non-Payment

If your account is over 90 days past due, you will receive a letter stating that you have 20 days to pay your account in full. Partial payments will not be accepted unless prior arrangements have been made with our office and are in writing. Please be aware that if a balance remains unpaid, we may refer your account to a collection agency and you may be discharged from the practice. If this is to occur, you will be notified by certified mail that you have 30 days to find alternative medical care. During that 30-day period, Dr. Wisniewski will only be able to treat you on an emergency basis.

Missed Appointments

Our policy is to charge for missed appointments not canceled within 24 hours. The charge will be $25 and will be your responsibility, billed directly to you. Please help us to serve you by keeping your regularly scheduled appointment or providing 24 hours notice when you have scheduling conflicts.

Prescription Refills

Three days prior notice is required for prescription refills. Please contact the front desk.

Phone Calls

Patient phone calls will be returned after 5:00 P.M. each day, and after 7:00 P.M. on Tuesdays. The front desk will not interrupt the doctor while he is seeing patients. Thank you for understanding our practice policies. Please let us know if you have any questions or concerns.
