Cal Arundel Family Medicine is a designated Carefirst Primary Care Medical Home.

Please bring your insurance card with you to each and every appointment. It is imperative that we confirm your continued coverage, and if you have a managed care plan, one of our doctors must be listed on your card (or be indicated by a unique identifying number). If you bring a card without this information, and have not made prior arrangements, you may be responsible for the full charge of your visit.

We also ask that you be prepared to pay any co-pay or amount due at the time of service. You may pay by cash, check or credit card.

If you have any questions regarding a bill you received for laboratory (blood, urine, etc.) studies, please direct them to the lab that processed the test.

Note that if your lab test was processed by LabCorp or Quest Diagnostics, you should direct questions regarding lab bills directly to the facility that did the work.

We recognize that insurance and billing have become extraordinarily complex. Insurance claims and patient billing are handled by Healthcare Practice Management. They will be happy to assist you. You can contact HCPM by calling 877-532-4482.

Please note: We assess a missed visit fee of $35 unless your appointment has been cancelled at least 24 hours prior to your scheduled time. Our patient's time is very important to us and we feel that arriving on time and being able to predict who will arrive is an integral part of the physician staying on schedule. In addition, it is frustrating to both patient and doctor when we have no appointments available, yet the physician is waiting idly for a no-show. Therefore, the missed office fee is invoked primarily as an incentive to properly notify us when you are not able to keep an appointment.
